At Dedicated Medical Care, LLC you must have your insurance card, state id card, and referral if required by your insurance, each time you come to our office. If you do not have proper proof of insurance you will need to reschedule your appointment or pay for the visit before you see the doctor.
It is the patients' responsibility to know and understand their insurance plan and requirements.
